Simple power of attorney
Document authorizing a third party (the apoderado) to handle the procedure on behalf of the owner. No notary required — just the owner's signature in front of two witnesses. It must include IDs of both the owner and the apoderado.
How to get it
Drafted by the owner. It must include: full names of both owner and apoderado, a specific description of the procedure being authorized, the owner's signature, and signatures of two witnesses with their info. Bring it printed to the appointment.
